The plan helps our patient’s budget for dental treatment and at the same time ensures you receive the same excellent quality of care.

- Regular hygiene visits to improve gum health and prevent gum disease which is the major cause of tooth loss.

- The essentials dental plan provides:

- Two dental assessments per year.
- All necessary x-rays (excluding CBCT).
- Two 30 min hygienist appointments per year (excludes children under the age of 18).
- Screening for oral cancer.
- On-going instructions on diet and oral hygiene maintenance.
- Emergency dental appointments during normal surgery hours, treatment fees (if required) apply.
- 10% discount on all other treatment fees*.
- Credit arrangements to spread the cost of any additional treatments.
- We provide free dental assessments for your children under 7 years of age.
- Worldwide emergency dental cover.
- Guarantee dental advice within 24 hours (during working hours).
- Membership for children – protecting their dental health

In addition to the adult benefits listed above, children also receive free fissure sealants in molar teeth and bespoke dietary advice.
Family Membership
The family discounts on the monthly membership fees are:
Please note that for family discounts all patients must live at the same address and fees must be collected by one direct debit.
- 2 people – 5%
- 3 or more people – 10%
* Fees for cosmetic, orthodontic, implant and any specialist treatment within the practice or referred out are not subject to the 10% discounted rate.
This discount cannot be used in conjunction with any other offer.
The plan joining fee is £8 and the monthly fee is £19.95.
Emergency Appointments
All patients who are members of the plan have access to all the necessary care and treatment in an emergency. We will endeavour to see you on the same day. Emergency appointments are available during normal surgery hours and treatment fees (if required) do apply.
How do I join?
It is very easy to join the plan, you can either complete the plan joining leaflet available at the practice and return to our reception team.
The minimum term for the plan is 12 months and you are able to join any time, if you would like your first examination at the practice to be inclusive of this plan then you will need to back date payment by 3 months.

- What is covered under the plan?

The Complete Care plan is designed to provide all routine* and necessary dental care to help avoid any large unexpected bills. It includes:
- Examinations.
- Hygiene treatments.
- X-rays.
- All necessary fillings.
- Preventative dental advice and
- therapy.
- Any necessary extractions.
- Crowns.
- Bridges.
- Dentures.
- Root canal treatment.
- Worldwide dental accident and emergency cover.
*at the discretion of your dentist.
- What is not covered?

Some treatments are not included in your plan, these are:
- Any referrals to a specialist, such as Implants, orthodontic. treatment and referrals for wisdom teeth extraction.
- Treatment deemed for cosmetic needs only e.g. tooth whitening.
- Dental laboratory fees.
- Prescription drugs.
- Sedation fees.
- Any treatment excluded from your individual contract.

- Guarantee – All dental treatment provided by the practice is guaranteed. The length and terms of the guarantee will vary according to the treatment but will be confirmed by the dentist.
How do I join?
After the initial assessment, where your dentist will assess the condition of your mouth, you will be placed into a category.
You will then be asked to complete a form and post it to the address printed at the bottom or simply hand the completed form to the receptionist.
Membership payments are collected monthly by Direct Debit.
There is a one-off joining fee of £8 and then your monthly fees are per band.

- Supplementary Accident and Emergency Insurance

All our dental plan patients (Essentials and Complete Care Plan) have Supplementary Insurance as part of the plan.
This is arranged for you to provide cover towards the cost of dental treatment in an emergency or following a dental injury, and can be used in the UK and abroad.
- What does my supplementary Insurance give me?

- Your Supplementary Insurance cover gives you:

- Emergency dental treatment when you are over 40 miles away from your registered dentists practice.
- Cover for out of hours consultations in the event of a dental injury or dental emergency.
- A hospital cash benefit if you are admitted overnight for dental treatment.
- Worldwide emergency dental treatment.
- Mouth cancer cover up to £12,000.

Your plan is unique to your registered practice. It is only transferable to another practice that is part of the Quality Dental Group.
If you need to move, please contact the practice and we will make alternative arrangements for you.
- Leaving your Plan

We understand that individual circumstances may change and that you may decide to leave the plans in the future.
It is a requirement to give three months notice to CODEplan in writing or by email.
